We are a team of registered mental health nurses who specialise in the care of older adults (over 65 years of age) with mental health problems living in the borough of Harrow.
We visit clients in the community, usually in their own home. Clients and their carers are encouraged to be actively involved in the assessment, planning, implementation and evaluation of their care.
We offer:
- Assessment of clients' present mental health needs
- Monitoring of mental health and progress with treatment
- Monitoring and advice with regard to medication
- Providing psychological therapies, for example counselling, cognitive behavioural therapy and anxiety management
- Offering advice, support and education to clients and carers
How to make a referral:
Most referrals are made to the multidisciplinary teams by general practitioners. Following referral you will be offered an assessment by one of the medical staff in the team. If it is thought that you would benefit from seeing a community mental health nurse, they will request that we come to see you to discuss how we can help.
